Before Leaving Your Country

     Flight Arrangements: Please arrange your return flight with destination to Ngurah Rai International Airport, Denpasar, Bali, Indonesia. As a participant of the QCS-1 Workshop, you may get special discount (75% discount for International Sector and 15% discount for Domestic Sector) from Garuda Indonesia, QCS-1 Official Airline. In order to redeem this special discount, you should go to the nearest Garuda Indonesia Office and show the invitation letter to Garuda Indonesia representatives. The special telex code for obtaining the discount is: CM/TX/217/APR02/RS with the subject: The 1st Intl Workshop QCS-1 in Bali.

     Accommodation and Airport Pickup Arrangements: Accommodation is available to participants from June 4-12, 2002. Please advise us if you wish to arrive earlier than June 4 or stay at the hotel after June 12.

     Airport transfer service is available from June 3-14, 2002. Kindly inform the contact person at the University of Surabaya (Ubaya) of your flight arrival/departure details including the flight number, date and time of arrival/departure so that arrangements for airport transfers can be made.

     Passport/Travel Documents and Visa Requirements: Foreign visitors to Bali must be in possession of valid passport/travel documents with a minimum validity of six months beyond the period of intended stay, confirmed return/onward tickets, sufficient fund to maintain themselves during their stay in Indonesia and visa (if applicable).      Health Regulations: Visitors arriving from Yellow Fever Endemic Zones, i.e. Africa or South America during the last 6 days are required to present International Health Certificate showing Yellow Fever Vaccination.
